Drug name: Berodual N
Active Ingredients:
- fenoterol hydrobromide
- ipratropium bromide
Therapeutic actions: Berodual N is a fix combination of two ingredients with broncholytic activity: ipratropium bromide is anticholinergic medicine, fenoterol hydrobromide is beta2- adrenoceptor agonist.
Ipratropium bromide exerts bronchodilating action due to anticholinergic properties. It retards the reflexes of vagus nerve, resists the effects of neurotransmitter acetylcholine released from nervus vagus. Anticholinergic agents prevent the increase of intracellular amount of cyclic GMP associated with interaction of muscarinic receptors with acetylcholine on the smooth muscles of the bronchi. Significant improvement of pulmonary function in people with Chronic Obstructive Lung Disease occurs in 15 minutes, maximal effect occurs in 1-2 hour and lasts 6 hours. Ipratropium bromide does not act on mucous secretion, mucociliary clearance and gas exchange.
Fenoterol hydrobromide exerts a direct sympathomimetic action. In therapeutic doses Fenoterol selectively stimulates beta-2- adrenoreceptors of the bronchi. In high dosages it can stimulate beta1-adrenoreceptors. Fenoterol relaxes the smooth muscle of the bronchi and vessels, prevents the development of bronchoconstrictive reactions caused by histamine, methacholine, allergens. Right after administration Fenoterol blocks the release of bronchoconstrictive inflammatory mediators from the mast cells.
Indications: Berodual N should be used in prophylaxis and treatment of obstructive diseases of airways with inversive bronchospasm:
- Chronic Obstructive Lung Disease
- Bronchial asthma
- Chronic bronchitis, complicated or uncomplicated by emphysema.
Contraindications and cautions: Berodual N should not be used in:
- Hypertrophic obstructive cardiomyopathy
- Tachyarrhythmia
- Hypersensitivity to any of the medication active ingredients
- Pregnancy(1 tremester)
- Children under 6 years of age
Berodual should be used with caution in:
- Closed angle glaucoma
- Hypertension
- Diabetes
- Recent heart attack
- Cardiovascular diseases
- Hyperthyroidism
- Pheochromocytoma
- Hypertrophy of the prostate gland
- Obstruction of neck of urinary bladder
- Mucoviscidosis
- Children
- Breastfeeding

The most common side effects include: tremor, nervousness, dry mouth, headache, dizziness, increased heart rhythm, cough.
The less common side effects include: nausea, vomiting, sweating, muscle pain, seizures. In very rare cases hypotension, irregular heart thuthm, angina(stenocardia), vision disorders, hives, swelling of the face, tongue, lips and face, anaphylactic shock.
