    Drug name: Climen

    Therapeutic actions: Menopause (change of life) is the cessation of menstruation, ie the time a woman has her last menstrual period; but the term menopause is used to define the years around that event when the body begins to produce less oestrogen.
    This dropping level of oestrogen may cause symptoms in some women.
    Climen is one form of oestrogen which is given to replace the oestrogen which the body is no longer producing.

    Indications: Climen is indicated in patients (with intact uteri) suffering from climacteric symptomatology.
    Climen therapy may be used as an adjunct in preventing osteoporosis in postmenopausal women.

    Contraindications and cautions: Climen is contraindicated in cases of: Pregnancy, lactation, severe disturbances of liver function, jaundice or persistent itching during a previous pregnancy, previous or existing tumours of the liver, uterus, ovaries or breast or a suspicion of such tumours, endometriosis, existing or previous thromboembolic processes, severe diabetes mellitus with vascular changes, sickle-cell anaemia, disturbances of lipometabolism, a history of herpes of pregnancy, otosclerosis with deterioration during pregnancy.
    Climen is not a hormonal oral contraceptive preparation.
    Strict medical supervision is necessary in patients with diabetes, hypertension, varicose veins, a history of phlebitis, otosclerosis, multiple sclerosis, epilepsy, porphyria, tetany, chorea minor.
    Reasons for immediate discontinuation of Climen: Occurrence for the first time of migrainous headaches or more frequent occurrence of unusually severe headaches, sudden perceptual disorders (eg disturbances of vision or hearing), first signs of thrombophlebitis or thromboembolic symptoms, a feeling of pain and tightness in the chest, pending operations (six weeks beforehand), immobilisation, onset of jaundice, onset of hepatitis, itching of the whole body, increase in epileptic seizures, significant rise in blood pressure, pregnancy.

    Adverse effects: Medicines and their possible side effects can affect individual people in different ways. The following are some of the side effects that are known to be associated with this medicine. Because a side effect is stated here, it does not mean that all people using this medicine will experience that or any side effect.

    • A feeling of tension in the breasts and menstrual irregularities (bleeding between periods, heavy periods) may occur, especially during the first few months of Climen. Changes of body weight and libido (sexual urge) occasionally occur.
    • Gastrointestinal complaints (nausea, vomiting, abdominal pain, flatulence), headache, ankle swelling and depressive moods occur rarely. Symptoms normally only last for a short time.
    • High blood pressure, back pain, muscle cramps, various skin reactions, itching and allergic reaction have been reported.
    • The following effects have been reported in users of different oral HRT preparations: painful periods, pre-menstrual like syndrome, indigestion, rashes, migraine, dizziness, anxiety and fatigue.
    • The frequency with which menstrual bleeding fails to occur in the tablet-free interval normally increases as the duration of treatment increases.
    • In rare cases benign and in even rarer cases malignant liver tumours ending in isolated cases to life-threatening internal bleeding have been observed after the use of hormonal substances such as those contained in Climen. The doctor must therefore be informed of the occurrence of unusual upper abdominal/stomach complaints which do not disappear of their own accord within a short time.
    • Climen should be immediately discontinued and your doctor consulted if you experience:

    Occurrence for the first time of migrainous headaches or an increase of unusually severe headaches, sudden perceptual disorders (e.g. disturbances of vision or hearing), first signs of thrombophlebitis or thromboembolic symptoms (for example, unusual pains in or swelling of the legs, stabbing pains on breathing or coughing for no apparent reason), a feeling of pain and tightness in the chest, onset of jaundice, onset of hepatitis, itching of the whole body, increase in epileptic seizures, significant rise in blood pressure.

    Interactions: It is important to tell your doctor or pharmacist what medicines you are already taking, including those bought without a prescription and herbal medicines, before you start treatment with this medicine.     Various substances (eg barbiturates, phenylbutazone, hydantoins, rifampicin) accelerate the metabolism of steroid hormones (possible impairment of effect); reduced substance levels have also been observed on concurrent use of some antibiotics (eg ampicillin) as a result of alteration of the intestinal flora.
    The requirement for antidiabetics or insulin can change as a result of an effect on glucose tolerance.
